    ASCEND is currently looking to fill the position of Restorative Residential Program Coordinator at our Community Living Apartments & Satellite Apartments..

    Restorative Residential Program Coordinator Job Description:

    Community Residences: Community Living Apartments

    The Community Living Apartments Program offers two types of living arrangements: Maple Street and Satellite apartments. Both include support and guidance through individualized service plans created in partnership with the service recipient. The first step in treatment is defining and developing a rehabilitation plan which aids a person's movement to the next stage in their recovery. This individualized, highly detailed plan clarifies needs, charts a course of action and provides the framework to evaluate progress. Services available to residents include crisis intervention, rehabilitation counseling, social and living skills development (money management, personal hygiene, food preparation, meal planning, recreation activities, etc.), symptom and medication management, community integration and resource development. On duty around the clock staff work closely with each resident to maximize stability and help in the transition to a less restrictive environment. Longer term assistance is available for people with problems preventing them from living outside a supportive setting.

    Individuals must be well motivated and committed to making a difference in people's lives. Good listening, writing and verbal skills required. The ideal candidate will be comfortable working independently.

    This is a full-time position, onsite in Hudson Falls, NY.

    Pay Rate: $62,500 - $65,000 per year

    Restorative Residential Program Coordinator Education & Qualification Requirements:

    • Master's or bachelor's degree in a human services related field; or
    • An associate's degree in a human services related field and three years' experience in human services; or
    • A high school degree and five years' experience in human services.
    • Valid NYS Drivers License

    Restorative Residential Program Coordinator Responsibilities include, but are not limited to:

    • Responsible for the day-to-day functioning and operation of the residence(s) including overseeing staff meetings, finances, supervision of activities and provision of services to residents.
    • Evaluate client referrals, conduct admission meetings w/supervisor and assist in the development of service plans and discharges for residents as needed.
    • Ensure and advocate that residents are receiving appropriate services with the program and that referrals are made for services outside the program when indicated, networking with community providers.
    • Implement and actively participate in selected program activities and be available to residents.
    • Participate in the development of program policies and procedures.
    • Be responsible for the required statistical and reporting documentation in the program (i.e. service plans, yearly reports, fire drills, program fees, etc.)
    • Encourage and facilitate resident input and participation in the development and maintenance of program offerings.
    • Provide and oversee orientation of new staff. Ensure timely compliance with all required trainings.
    • Provide regular supervision to and evaluation of Assistant Coordinators, Care Plan Specialists, Benefits Specialists, Residential Specialists, and Relief Residential Specialists.
    • Oversee the physical condition of the residence(s); make/approve small repairs as needed, inform supervisor of larger maintenance problems.
    • Be On-Call as assigned.
    • Assist in OMH certification/re-certification processes as required.
