    About Us

    Healthy Alliance connects community members to a growing social care network (SCN) of organizations — big and small — that provide services that are essential for a healthy life. Recognizing that health begins in our communities, Healthy Alliance convenes and collaborates with all parts of the health care ecosystem — from regional health systems and federally qualified health centers to grassroots clothing and household goods providers and everyone in between — to address health-related social needs like food insecurity, housing assistance, transportation needs, benefits navigation, and much more before they evolve into serious and costly medical problems. As the official SCN lead entity in the Capital Region, Central New York, and North Country under New York’s 1115 Waiver Demonstration Amendment, New York Health Equity Reform (NYHER), Healthy Alliance has 1,300 partner locations spanning 24 counties across their three regions – working to provide all communities with consistent access to the resources they need to ensure every New Yorker has the same opportunity to be healthy.
