Director of Health Home Care Management
Posted: 01/10/2025ASCEND is currently looking to fill the position of Director of Care Management. This individual will oversee our Health Home Care Management Program at our site in Glens Falls, NY.

Duties include, but are not limited to:
- Responsible for the day-to-day operation of the Care Management programs, including staff scheduling, finances, supervision of activities, scheduling and program offerings and provisions of services to clients.
- Ensure smooth functioning of the programs, delegate tasks to all staff as needed to ensure functioning in line with Agency and policies and procedures.
- Supervise and coordinate the work activities of direct reports.
- Interview and hiring of new staff.
- Evaluate referrals and assign them to appropriate programs.
- Ensure that client records and program records are adequate and current.
- Ensure that NYS Office of Mental Health , the Department of Health, and the Health Home Lead Agency regulations are followed.
- Ensure that clients receive appropriate services within the programs and that referrals are made for services outside the programs when indicated.
- Implement and actively participate in selected program activities; be available to individual clients as appropriate.
- Assist in the development of program policies and procedures, evaluate procedures for effectiveness, and ensure that program policies and procedures are adequate and appropriate.
- Plan and monitor use of the program budget in conjunction with the Chief Financial Officer of the Agency.
- Be responsible for statistical and other reporting of programs to the Agency and to other appropriate bodies such as the NYS Office of Mental Health, the Department of Health, and the Health Home Lead Agency.
- Represent the Agency and the program, maintain good working relationships with other agencies, services and individuals in the community, with other service agents, facilitate coordinated service provision for clients.
- Develop, implement and monitor program services according to the changing needs of clients, evaluate service delivery of the program and its components on a regular basis.
- Encourage and facilitate client input and participation in development and maintenance of program offerings.
- Coordinate and facilitate staff meetings.
- Ensure that the environment and its fixtures are in good working order and that safety procedures including fire safety are followed and that safety equipment is fully functional.
- Attend Agency meetings as determined by supervisor, such as Directors/Leadership Meetings, Incident Review Committee, Corporate Compliance Committee, etc.
Education Requirements:
- Bachelor’s degree with case management experience that includes a significant amount of supervisory/leadership experience, OR has an Associate’s degree with NYSHCBS Supervisory waiver with 5 years case management experience with a significant amount of that experience including supervisory/leadership experience.
- Valid NYS driver’s license acceptable to Agency insurance standards.
- Must be of good character and background and display an affinity for working with people with mental illness and/or co-occurring disorders.
- Must have the ability to work independently, have good problem-solving skills and can work as part of a multi-disciplinary team.
