Insurance Rates and Benefits for 2009 -

The Adirondack Regional Chamber of Commerce does not set the insurance rates and assumes no responsibility for their accuracy or any increases that may occur. The rates were provided to ARCC by CDPHP as of 6/23/08 and became effective on 7/1/09. Blue Shield rates were provided on 11/10/09 and will become effective on 1/1/10. MVP rates were provided on 11/18/09 and will become effective on 1/1/10.

The plans we offer are called 3-Tier plans, which means that you can select from Individual, 2-Person or Family coverage. As allowed by law, the insurers charge higher rates for sole proprietors than small employers with employees numbering from 2-49. The following is the waiting period requirement for all health and dental plans:

• Sole Proprietors must be an ARCC member for 60 days prior to becoming eligible for health insurance.

• Groups of 2-49 must be an ARCC member for 30 days prior to becoming eligible for health insurance.

To Note:

CDPHP, Blue Shield and MVP offer domestic partner coverage. Additional forms and documentation are required by those insurers, which must be completed and sent in with your enrollment application.

To apply for coverage under one of these plans and to learn about rates, please contact the ARCC at 5 Warren Street, Glens Falls, NY 12801, 798-1761, dzeman@adirondackchamber.org. The appropriate tax documentation is required along with your insurance application paperwork. This is an insurance carrier, not ARCC, requirement. Application does not guarantee acceptance by the insurance provider.

Payment:

There is a one-time $15 new enrollment fee for each new health or dental application. ARCC bills quarterly, in advance. There is a $20 quarterly administrative fee per policy holder.

If you prefer to pay your premiums on a monthly basis, the ARCC does offer members the options of doing so via an automated debit receipt from your checking or savings account. To take advantage of this monthly payment option, please complete this ACH Authorization and forward it to Deb Zeman.
